We’ve been having some issues uploading the PDFs to the website, but we’ve created Google Documents for the two chaperone forms. If you are bringing your own child or grandchild, or your legal partner’s child or grandchild, you do not need to fill out the form (but there’s no downside in doing so). If you are the chaperone for a minor, the parents of that minor need to sign fill out and sign one of these forms, and you need to bring it to registration; the minor in question cannot pick up his or her registration packet without it.
